Testicular Cancer Blood Screening

Testicular cancer is the most common malignancy in men aged 15 to 44, with approximately 2,300 new cases diagnosed annually in the UK. It has one of the highest cure rates of any cancer, exceeding 95% when identified at an early stage, making awareness and timely investigation clinically important.

This screening measures three established serum tumour markers associated with testicular germ cell tumours: Alpha-Fetoprotein (AFP), Beta-HCG (Human Chorionic Gonadotrophin), and Lactate Dehydrogenase (LDH). AFP is characteristically elevated in non-seminomatous germ cell tumours, while Beta-HCG may be elevated in both seminomas and non-seminomatous tumours. LDH is a less specific marker but may correlate with tumour burden.

It is important to understand that tumour markers contribute to clinical assessment but are not diagnostic in isolation. Normal tumour marker levels do not exclude testicular malignancy, and mild elevations can occur in non-malignant conditions. Definitive assessment requires clinical examination and scrotal ultrasound alongside tumour marker measurement.

This screening is appropriate for men with established risk factors including a history of cryptorchidism (undescended testis), family history of testicular cancer, or previous contralateral testicular malignancy. It is also suitable for men who have noticed a testicular mass, swelling, or asymmetry, or those who wish to establish baseline tumour marker values. For men who have completed treatment for testicular cancer, regular tumour marker monitoring is an essential component of post-treatment surveillance, as directed by their oncology team.

The blood sample is collected by our registered nurse at our City of London clinic. All samples are processed by a UKAS-accredited laboratory. This screening provides laboratory data — it does not replace testicular self-examination or clinical assessment.
