Male Hormonal Health Screening
This panel provides a comprehensive assessment of the male hormonal axis through measurement of testosterone, Sex Hormone Binding Globulin (SHBG), gonadotrophins (FSH and LH), prolactin, and calculated Free Androgen Index. These markers collectively evaluate the hypothalamic-pituitary-gonadal axis and help identify hormonal imbalances that may underlie clinical symptoms.
Testosterone is the primary male sex hormone, essential for muscle mass, bone density, fat distribution, erythropoiesis, libido, and spermatogenesis. However, total testosterone alone can be misleading — SHBG is a binding protein that determines how much testosterone is biologically active. When SHBG is elevated, bioavailable testosterone may be low even when total testosterone appears within the reference range. The Free Androgen Index, calculated from the ratio of testosterone to SHBG, provides a more clinically useful estimate of bioavailable testosterone.

FSH and LH are pituitary gonadotrophins that regulate testicular function. Measuring these alongside testosterone helps distinguish primary hypogonadism (testicular origin, characterised by elevated FSH and LH) from secondary hypogonadism (pituitary or hypothalamic origin, characterised by low or inappropriately normal FSH and LH). This distinction is clinically important as it guides further investigation and management approach.
Prolactin is included because hyperprolactinaemia can suppress gonadotrophin release and contribute to hypogonadism and erectile dysfunction. Significantly elevated prolactin may warrant pituitary imaging.
This screening is appropriate for men experiencing symptoms such as fatigue, reduced libido, erectile dysfunction, low mood, reduced muscle mass, or those seeking a baseline hormonal evaluation. The blood sample is collected by our registered nurse at our City of London clinic. Morning sampling is recommended as testosterone exhibits diurnal variation with peak levels in the early morning.

Ensure adequate sleep the night before — poor sleep can transiently lower testosterone.
