About This Screening
Female Hormonal Health Screening
This panel provides a comprehensive assessment of female hormonal status through measurement of key reproductive and endocrine hormones. It includes oestradiol, FSH, LH, prolactin, testosterone, Sex Hormone Binding Globulin (SHBG), and the calculated Free Androgen Index.
Female hormones fluctuate throughout the menstrual cycle and across the lifespan, which makes the timing of sample collection clinically important. For general hormonal assessment, sampling on days 2–5 of the menstrual cycle (early follicular phase) is standard. For ovulation confirmation, day 21 (mid-luteal phase) progesterone measurement is appropriate. Our nursing team can advise on optimal timing based on your clinical question.
Key Highlights
- Assess oestrogen and androgen balance
- Evaluate FSH and LH for ovarian function assessment
- Measure testosterone levels (clinically relevant in women)
- Screen for hormonal patterns associated with PCOS
- Assess perimenopausal or menopausal status
FSH and LH are pituitary hormones that regulate ovarian function. The FSH-to-LH ratio assists in evaluating conditions such as polycystic ovary syndrome (PCOS) and menopausal transition. Elevated FSH may indicate diminished ovarian reserve, while the pattern of FSH and LH elevation or suppression helps distinguish different hormonal conditions.
Testosterone and SHBG are included because androgen excess is a key feature of PCOS and can present with acne, hirsutism, or alopecia. The Free Androgen Index provides a more clinically useful measure of androgen activity than total testosterone alone. Prolactin measurement screens for hyperprolactinaemia, which can cause menstrual irregularity, galactorrhoea, and impaired fertility.
This screening is appropriate for women with menstrual irregularities, suspected PCOS, perimenopausal symptoms, fertility concerns, or those seeking a baseline hormonal evaluation. It is important to note that hormonal contraception suppresses endogenous hormone production — results must be interpreted in this context and ideally testing occurs after a washout period.
The blood sample is collected at our City of London clinic by a registered nurse. Results are processed by a UKAS-accredited laboratory and reviewed by our clinical team.
