When do wisdom teeth need removing?
Wisdom teeth — also called third molars — typically emerge between the ages of 17 and 25. Not all wisdom teeth require removal. If yours have fully erupted, are properly aligned, healthy, and accessible for cleaning, they can usually be left in place. Extraction is recommended when teeth are impacted (trapped beneath the gum or bone), partially erupted, causing recurring infection (pericoronitis), damaging adjacent teeth, or contributing to crowding.
At St Paul's Medical & Dental in The City of London, we assess each case individually with X-rays to determine whether extraction is clinically appropriate. We do not recommend extraction without a clear clinical indication.
Types of wisdom tooth extraction
The type of extraction depends on how the tooth is positioned in your jaw. A simple extraction applies to fully erupted teeth that can be removed with forceps — priced at £250 (£125 for members). A surgical extraction is required when the tooth is partially erupted and the dentist must make a small gum incision — £450 (£225 for members). A complex extraction involves fully impacted teeth trapped within the jawbone, often requiring bone removal — £670 (£335 for members).

What to expect during the procedure
Wisdom tooth extraction is performed under local anaesthetic to numb the treatment area. You may feel pressure and movement during extraction, but you should not feel sharp pain. Individual experiences vary depending on the complexity of the case and personal sensitivity. The procedure typically takes between 15 and 60 minutes per tooth.
For straightforward cases, the dentist loosens the tooth with an elevator instrument and removes it with forceps. For surgical or complex cases, a small incision is made in the gum, and in some cases a small amount of bone may need to be removed. Dissolvable or removable sutures may be placed to close the site.
Risks and complications
As with any surgical procedure, wisdom tooth extraction carries risks. These may include:
- Dry socket (alveolar osteitis) — where the blood clot is lost from the extraction site, causing increased pain. More common with lower wisdom teeth.
- Infection — post-operative infection can occur and may require antibiotics.
- Nerve injury — temporary or (rarely) prolonged numbness of the lip, tongue, or chin due to proximity of the inferior alveolar or lingual nerves.
- Bleeding — some bleeding is normal; prolonged bleeding is uncommon but possible.
- Swelling and bruising — expected after surgical extraction and usually resolves within 7–10 days.
